A long-awaited autopsy report on the death of rapper Kanye West's mother did not determine a precise cause of death but did generate new questions about the medical care she received both before and after undergoing plastic surgery Nov. 9 in Brentwood. Louis A. Pena, a Los Angeles County deputy medical examiner, wrote that Donda West "died from some preexisting coronary artery disease and multiple postoperative factors following surgery."

More than a year after the death of rapper Kanye West's mother, state officials are investigating her nephew, a registered nurse who oversaw her recuperation after her plastic surgery, according to a person with knowledge of the probe. Stephan Scoggins, who received his nursing license in 1990 and has an advanced degree in public health, was in charge of caring for Donda West when she returned home Nov. 9, 2007, after a 5 1/2 -hour operation.

The mother of rap star Kanye West, who died earlier this month after undergoing routine plastic surgery, likely succumbed to a heart attack, pulmonary embolism or accidental overdose of drugs prescribed to her for pain after the procedure, her surgeon said in an interview Monday. In his most detailed public comments on the case, Dr. Jan Adams told The Times that nothing went wrong Nov. 9 when he performed a breast reduction, tummy tuck and liposuction on 58-year-old Donda West.
