Alleged Health Scam Halted: The state Insurance Department said it shut down an allegedly fraudulent health insurer that operated out of an Ontario post office box and was collecting premiums of more than $200,000 a month for worthless medical coverage. The company, United Healthcare Benefits Trust, and its founder, Tony Ullah, 46, of Upland, originally were ordered to halt operations in March, 1990.
